Your First Visit:

Thank you for choosing Family Medicine of SayeBrook. We look forward to caring for you and your family. To make your first visit with us as quick and comfortable as possible, it is important to:

1. Bring your insurance card or cards.
2. Bring a valid picture ID.
3. Have your records forwarded to our office. You can download a Medical Records Release Form from the 'Patient Forms' area to the left.
4. Fill out your new patient forms. Download all 7 forms from our 'Patient Forms' area to the left.
5. Bring your current medications or a list of your medications to your visit.
6. Come 30 minutes prior to your scheduled appointment.
