Thank you for choosing Family Medicine of SayeBrook. We look forward to caring for you and your family. To make your first visit with us as quick and comfortable as possible, it is important to:

1. Bring your insurance card or cards.
2. Bring a valid picture ID.
3. Have your records forwarded to our office. A medical release form is part of the patients forms found in the area to the left.
4. Fill out your new patient forms. Download all 9 forms from our 'Patient Forms' area to the left.
5. Bring your current medications or a list of your medications to your visit.
6. Come 30 minutes prior to your scheduled appointment.
